Medication Assisted Treatment in Illinois Corrections: A Journey of Growth and Hope

In 2004, I was working as a “Chemical Dependency Counselor” on the west side of Chicago. Sheridan Correctional Center, in Sheridan, IL. had recently reopened as a fully dedicated substance abuse disorder treatment facility. During that time, I was honored to work for anorganization that received one of the first groups of men released after completing almost a year of treatment inside Sheridan.


Upon their release, they weren’t thrilled to hear they had another 12 weeks of programming (with me) to fulfill their probation obligations. Still, most of them showed up and determined to cross the finish line. Out of my 24-client caseload, several had a diagnosis of “opioid abuse” and it didn’t take long to see how much they would struggle to maintain their recovery after release.


At the time, Medication-Assisted Treatment (Now known as Medication Assisted Recovery in IL.) was still in its infancy in Illinois corrections. It was something we discussed sparingly, and resources were limited. But I knew I had an obligation to understand the evolving landscape around MAT and how it could impact the men I worked with. I didn’t realize it then, but those early days were the foundation of my work in helping to launch Buprenorphine in IL., and reentry, journeys that continue to this day.


Illinois’ Journey with MAT: A Timeline of Progress


Since 2004, Illinois has made significant strides in integrating MAT into its correctional system. Each milestone reflects the state’s commitment to improving health outcomes and reducing recidivism among justice-involved individuals.


Sheridan Correctional Center: Pioneer of a New (National) Model


2004: Sheridan Correctional Center reopened as Illinois’ first correctional facility fully dedicated to substance use disorder treatment, utilizing a therapeutic community model.


Cook County Jail: MAT Services


2014: Cook County Jail became the first in Illinois to offer MAT services, paving the way for other facilities. Learn more about Cook County Jail’s Opioid Treatment Programming here:


2016: The jail began distributing naloxone (a.k.a Narcan) upon release, equipping individuals with life-saving overdose prevention tools. For free Naloxone in IL., please visit:


2019: Cook County Jail joined a national initiative to expand MAT services in correctional facilities, further addressing the opioid epidemic.


Statewide Expansion of MAT


2018: A survey of 36 Illinois jail administrators revealed varying levels of MAT implementation. For IL Jail MAT survey findings:


2019: Injectable naltrexone services were introduced in 10 county jails and Sheridan Correctional Center, expanding access to evidence-based care. For MAT Services in IL:


2022: The Illinois Department of Human Services launched the Illinois Learning Collaborative to Support Medication-Assisted Recovery (MAR), providing technical assistance for justice-involved populations. For MAR progress reports:


2023: By March, 19 counties (excluding Cook County) had joined the collaborative, representing 46% of the state and reflecting significant progress in MAT expansion.


The Work Ahead


While we’ve come a long way, challenges remain in ensuring equitable access to MAT across all correctional facilities. To truly combat the opioid crisis, we must:


Expand Access: Ensure MAT services are available in all correctional facilities statewide.


Educate Staff: Provide training to correctional staff on MAT to reduce stigma and encourage participation.


Increase Community Partnerships: Strengthen collaborations between corrections, treatment providers, and community organizations for seamless reentry support.


Focus on Equity: Address disparities in access to treatment for underserved populations, particularly communities of color.


Craig’s Journey


One of my most challenging clients during those early days serving Sheridan clients was a gentleman I will refer to as “Craig”. He was skeptical of MAT and counseling with me. He was hesitant to trust anyone, but he stuck with me and began MAT per my encouragement. The more that I learned, the more that I taught Craig and the other clients. Craig completed his 12 weeks with counseling and Buprenorphine, I saw him transform.


After finishing the program, he began working as a barber and eventually saved enough to open his own barbershop. Not only did Craig build a thriving business, but he also made it a mission to hire other men reentering the community, offering them the same second chance he had been given.


Craig didn’t stop there. He got married and had twin daughters, who he proudly brought to visit me whenever he could. We lost touch around 2012, but I trust that Craig and his family are continuing to thrive, just as we dreamed.


Craig’s story is a testament to the power of second chances and the impact that dedicated correctional programs can have.
